BMI: What It Means and Its Limitations
What Is BMI?
Body Mass Index (BMI) is a simple screening tool that estimates body fat based on your height and weight. The formula is: BMI = weight (kg) / height (m)^2. It was developed in the 1830s by Adolphe Quetelet as a population-level statistical tool, not as an individual diagnostic measure.
BMI Categories
- Underweight: BMI below 18.5
- Normal weight: BMI 18.5 to 24.9
- Overweight: BMI 25.0 to 29.9
- Obesity Class I: BMI 30.0 to 34.9
- Obesity Class II: BMI 35.0 to 39.9
- Obesity Class III: BMI 40.0 or higher
These categories were established by the World Health Organization and are used globally as a first-pass screening tool.
What BMI Gets Right
At a population level, BMI correlates reasonably well with body fat percentage and health outcomes. People with a BMI over 30 statistically face higher risks of type 2 diabetes, cardiovascular disease, certain cancers, and other conditions. It remains useful because it requires no special equipment — just a scale and a measuring tape.
Important Limitations
BMI does not distinguish between muscle and fat. A muscular athlete may have a BMI of 28 with very low body fat, while a sedentary person with the same BMI might carry excessive fat. BMI also fails to account for:
- Body composition — It says nothing about where fat is distributed. Visceral fat around the abdomen is more dangerous than subcutaneous fat.
- Age and sex — Older adults and women tend to have more body fat at the same BMI.
- Ethnicity — Health risks at given BMI levels vary across populations. Some guidelines use lower thresholds for people of Asian descent.
When to See a Doctor
BMI is a starting point, not a diagnosis. If your BMI falls outside the normal range, or if you have concerns about your weight regardless of your BMI, consult a healthcare provider.
